a) Mail in the Application Form and all of the applicable documents noted below.
b) Submit the Application Form when the parents/guardians at the time of the school visit and mail in the documents noted below at a later date.
c) Mail in Application Form before or after the school visit. The following documents need to be mailed with the Application Form:

• Transcript (Not required for Kindergarten Applicants)
• Recommendation Letters (Not required for Kindergarten Applicants)
  (To be filled out by previous teachers and sent directly back to us.)
• Entrance Medical Clearance
• Health Questionnaire
• Agreement of Payment
• Bus Application Form (If interested in school bus service)
  
  *Please complete all the required documents in English (or romaji) wherever possible.*

The interview with the Principal will be arranged ONLY after all of the necessary documents have been completed and received by the school. The Admissions Officer will then contact the child's parents/guardians to set the date for the principal interview and the placement test.

Please be sure that the all of the necessary payments, including tuition, are made by the date stated on the invoice. If the Accounting Office is unable to confirm payment, A-JIS may cancel the applicant's acceptance to the school.
